There was a reason that we had four fail-safe mechanisms to prevent an accidental explosion. From the original posted article, it looks like the military decided to go back to the drawing board to redesign those safety mechanisms after three of the four failed.

Close call, to be sure, but there was nothing drunk or reckless about our approach to designing fail-safe protections in our nuclear weapons.

I also watched a recent PBS special about the Cuban missile crisis, and little did we know that we almost had a nuclear war due to the isolation of the Russian nuclear fleet from Russian command. An aggressive commander of the fleet actually ordered a nuclear strike, only to be countermanded by his next in line.
