When it's something like a funeral or such as your situation with someone going in for surgery, serious issues like that, you just shut the fuck up.

a) It's not about you.
b) It's not the time and place to have such a discussion.

I agree with muffs. They are looking for support and comfort. It is not going to hurt you one bit to send a little. You do not have to agree to pray, but you can send them back a "keeping in my thoughts", "hoping for a sucessfuly surgery", etc text. They will feel better and you have not compromised.
