For a setting which was 'Low magic, high grit, realism (As well as lacking in sanitation, medicine etc) look up Harn World.

No dragons, Wizards were few and far between. If the GM placed things right, players need never even meet a Dwarf, Elf or Orc.

Plus, the castle and city designs were top notch.

Think of it like Shadowrun, except in the present instead of Cyberpunk. Only in the default setting, the big twist is that most of the world simply can't perceive the magical from the mundane, and so there is this almost Matrix like quality to the world. There is a relatively small amount of people who are 'in the know' and can see the world of magic, elves, and dragons that is reemerging for what they truly are; everyone else suffers from a cognitive dissonance so severe that their brains effectively make things up to cover for what they really see (the dragon attack was a gas main explosions, the group of goblins was really a gang of ugly midgets, etc.). They (the 99%) are collectively refereed to as mundanes.

The game's systems are stock D20, so basically D&D 3.5 but in a low magic, high tech modern setting. Less fireballs, fullplate, crossbows, and spell-books. More arcane hacking, bullet-resistant vests, assault rifles, and smartphones.

Plus there are dapper Bugbears, how about that?

Old school analogue RPG, a football injury at school led me to a nerd den in 1987, D&D and Mechwarrior: The Battletech Role-Playing Game. I miss that imagination driven content you can't get from computer games.
